If you can get it, ask your regular doctor or specialist to write a brief letter confirming your diagnosis and describing your condition. The letter should mention:

- Your response to those treatments
This letter is not required. When you book your appointment with MarijMD, you can ask whether you should try to get this letter before your visit.
How to Submit Your Records
Most patients have two options: upload your documents before your appointment through the online booking system, or bring physical copies with you on the day.
Uploading ahead of time allows the MarijMD team time to review everything before you arrive. If you can’t upload, bring clear, legible copies (or originals if you have them) of anything that documents your condition and your medical history.
You’ll Need a Credit Card to Hold Your Appointment

When you book your evaluation appointment online, you’ll be asked for a credit card to secure your reservation. Have your card ready when you’re booking. The MarijMD team will let you know the deposit amount when you schedule.
A Valid Photo ID and Proof You Live in Florida
Bring a government-issued photo ID (driver’s license, passport, or state ID). You’ll also want proof that you live in Florida, such as a recent utility bill, lease agreement, or mortgage statement. This confirms you’re a Florida resident, which is a requirement for the state medical cannabis program.

What Happens During Your Appointment

If the physician determines you qualify, they’ll enter you into Florida’s Medical Marijuana Use Registry (MMUR) and provide documentation you’ll need to apply for your state ID card with the Department of Health.
